Appointments
Office visits are by appointment, so please call in advance for your appointment. This will allow us to reserve a time especially for you. We find this works to everyone’s best interest, so Dr. Hart can spend the time that is necessary for each individuals needs. We do, however, allow for emergency times, when our patient’s have special needs.
We make every effort to be on time for our patients, and we ask that our patients extend the same courtesy. If you cannot make an appointment, please notify the office as soon as possible. This courtesy will allow us to give the appointment to another patient who might be in need.
New Patient Forms
We have made our new patient forms available to download and print (PDF format). If you have any problems, please call the office and we'll be glad to send or fax them to you.
Once these are filled out, you will be able to submit them to the Practice before your scheduled appointment by fax or email or just bring them with you for your first appointment.
Insurance
Our staff is very knowledgeable in this area, and we will do our best to help our patients maximize their benefits according to individual needs. We will do our best to assist you with processing your insurance claims. As a courtesy to our patients we will complete the necessary claim forms and send them to your insurance company, when you come for a visit. We will wait a reasonable amount of time for payment from your insurance company; however the financial commitment to this practice is the patient’s responsibility.
